Registration of the Death


All deaths must be registered with the Registrar of Births and Deaths for the area in which the person died. The address and telephone number of the Registrar can be found within this information pack and an appointment with the Registrar to register the death should be made at the earliest opportunity. If a death has not been referred to the Coroner then it must be registered with the Registrar within 5 days of the death occurring.

Before attending your appointment with the Registrar you should check who should attend your appointment as someone other than you may be required to sign the register of deaths.

When you meet with the Registrar you should take the following information and documentation with you:-

  • The deceaseds full name and maiden name if applicable and their last address
  • Details of their place of birth
  • The deceaseds occupation and if applicable, the occupation of her husband
  • If the deceased was married, then the date of birth of the surviving partner
  • Details of any pension the deceased was receiving from the State, the Services or Public Funds
  • The Medical Certificate detailing the cause of death
  • The deceaseds medical card if available
  • The Pink Form (Form 100) if you have been given one by the Coroner 
The Registrar will issue you with:-

  • A Certificate for Burial or Cremation known as the Green Form. That is unless the Coroner has already issued a Burial Order (Form 101), or a Cremation 6 (Crem.6 form).
  • A Certificate of Registration of Death (Form BD 8 (rev)). This form is for Social Security purposes and if applicable should be completed and sent to the Social Security Office.
